A shrine located in the vicinity of the church of St. Anna in Zaborów, constructed by Jan Kujawski, a teacher from the local school, who conducted classes in Polish before the First World War (despite the objection of the tsarist authority). For the 10th anniversary of regaining independence by Poland in 1928, Jan Kujawski together with his students decided to celebrate the round anniversary by building the shrine. They formed a small mound on top of which they placed a pedestal with an eagle and masoned in a commemorative plaque. In 1932 the heirs of the properties in Zaborów and Zaborówek – Zofia Goldstand and Gabriel Wodziński placed a statue of Our Lady of Perpetual Help in the shrine.
